About the Practice | Grant Street Medical Centre is a family-orientated practice. We are committed to providing our patients with the best quality medical care in an environment that is warm, caring, safe, professional and respectful. We are a fully accredited practice with GPA. We pride ourselves in achieving and maintaining the highest level of patient care. |
Home Visits during Surgery Hours | Home visits may be available for regular patients who live locally within 5Km radius and are too ill or frail to attend our practice. These visits are generally performed during lunchtime or after the clinic has closed in the evening. Please contact us on 03 4367 7555 if you require this service. In addition, GSMC provides telehealth appointments for the patients who are not able to attend the clinic in person. Note: To be eligible for telehealth appointments, patient must have been to GSMC in person within the last 12 months period. You are welcome to confirm your previous visits with our reception team by calling us. |
Electronic communication | Grant Street Medical Centre's primary reason for communicating electronically to patients is to issue appointment reminders and recalls and we verify the correct contact details of the patient at the time of the appointment being made. Patient consent needs to be obtained before engaging in electronic communication. Electronic communication includes Email, Facsimile and Short Message Services (SMS). |
After Hours Home Visits: | For all after hours home visits, please call Bacchus Marsh Hospital Emergency on 5367-9195. For all critical emergencies, please call "000" immediately. |
Appointments and Walk-In Patients: | Appointments are required for all the consultations and each appointment is for one patient only. Grant Street Medical Centre welcomes walk-in patients but depends on the availability. Please contact the clinic to see if we can accommodate walk-in patients on the day. Please let the receptionist know if you would like to book a long appointment or if you need to book for additional family members. Sometimes delays can occur due to emergencies and other circumstances beyond our control but we do our best to minimize your waiting time. If you are unable to attend your appointment please phone the clinic to cancel so that we can offer that appointment time to another patient. |
Telephoning your doctor: | Doctors in this practice may be contacted by phone during normal surgery hours. If the doctor is with a patient, a message will be taken, and your call returned as soon as possible. If your call is urgent, you will always be put straight through to the doctor. |
Referrals: | Referrals to a specialist may be made during the consultation. Standard referrals are valid for one year so please ensure you book an appointment before your referral runs out. |
Prescriptions: | Prescriptions may be written during a consultation. If you need repeat prescriptions, we ask that you make an appointment to see one of our doctors, where the condition for which the prescriptions are written can be checked. |
Results: | Sometimes your doctor will order investigations such as blood tests or x-rays. For confidentiality reasons, please book in for a return appointment to discuss the results. |
Reminder System: | Our practice is committed to preventative care. You will receive reminder notices for health care services appropriate to your care. If you do not want this service, please let the doctor know.
PLEASE NOTE: Patients need to pay Doctor's consultation fee and procedure fee if you are coming for any Treatment Room procedure. |

This can involve in the collection of patient data. However, this information is de-identified and those involved in analysis of the data would not be able to identify which patient has been included.
